Member Services Baby Dedication Form

PLEASE NOTE THE FOLLOWING:

• Baby Dedications are reserved for members only. 

• Baby Dedications are for infants and toddlers up to 2 years old.

• Baby Dedications must be scheduled a month in advance and will be during the 10:30AM Worship Experience unless otherwise noted.

• Baby Dedications are scheduled on a first-come/first-served basis. There is no guarantee that the month you have requested will be available. However, you will be scheduled for the next available ceremony.
